Before you begin: You may need appropriate PS Suite permissions to add a custom form. If the options described below are unavailable, ask your clinic's PS Suite administrator for assistance.

Visit the LifeHeart Cardiology Website

Go to the LifeHeart Cardiology referral page and look for the TELUS PS Suite .cfm download. The filename will clearly identify it as the LifeHeart Cardiology Referral Form.

What is a .CFM file?
A .cfm file is a custom form designed for TELUS PS Suite. Unlike a regular PDF, a properly configured PS Suite form can use information already stored in the patient's chart, helping your clinic prepare referrals more quickly and accurately.

Download the Form to Your Computer

Click the download button and save the .cfm file to your computer. In most cases, it will be saved automatically in your Downloads folder.

Can't find the file later?
Your PS Suite environment may not have permission to access the Downloads folder. If that happens, move the file into a folder PS Suite can access, such as Documents, My Documents, or a clinic-approved shared folder. Keep the .cfm file in its original format — do not rename or convert it before importing.

Open the Records Window in PS Suite

Sign in to TELUS PS Suite and open the main Records window. You do not need to enter any patient information during the initial installation. The form is being added to your clinic's available custom forms so it can be used later from individual patient records.

Open the Custom Forms Area

From the PS Suite Records window, select:

Edit → Forms

This will open the area where your clinic's available custom forms are managed.

Menu may vary: Depending on your clinic's PS Suite version or configuration, the wording may be slightly different. Some installations may display an Edit Custom Forms option under Settings.

Drag the .CFM File into the Forms Folder to Import

In the Forms window, select Add. Browse to the folder where you saved the LifeHeart Cardiology .cfm file — usually:

Downloads → LifeHeart Cardiology Referral Form.cfm

Select the file and follow the on-screen prompt to add or import it.

Folder access issue? If PS Suite cannot see your Downloads folder, move the file to Documents, My Documents or a clinic-approved folder and try again.

Confirm the Form Was Added

Once the import is complete, look through the available custom forms and confirm that the LifeHeart Cardiology Referral Form appears in the list.

Quick win: Let other physicians and administrative team members know the form has been installed and what name to search for in PS Suite.

After the one-time installation is complete:

Open the patient's record in PS Suite.

Always confirm that the correct patient record is open and review all auto-populated information before sending the referral.

You're Ready to Refer

Once installed, the LifeHeart Cardiology form becomes part of your clinic's PS Suite workflow. There is no need to download a new copy for every patient.

Open patient → Select form → Complete referral → Review → Fax

Troubleshooting

The file downloaded, but I cannot find it
Check your browser's Downloads list or your computer's Downloads folder. If necessary, download the file again and choose a folder you can easily locate.
PS Suite cannot access my Downloads folder
Move the .cfm file into Documents, My Documents or a clinic-approved shared folder. PS Suite access permissions can differ between clinics and workstations.
I do not see the Forms or Add option
Your PS Suite account may not have permission to install custom forms. Ask your clinic's PS Suite administrator or technical support contact to complete the import.
The patient information did not populate
Auto-populated fields depend on how information is stored and configured in your clinic's PS Suite environment. Review the patient's demographics and complete any missing information manually.
The fax number did not populate
Confirm that LifeHeart Cardiology has been added to your PS Suite address book with fax set as its default delivery method: +1 647-255-1636.
The menus look different in our version of PS Suite
PS Suite configurations and menu labels can vary. Your clinic may access custom forms through Settings → Edit Custom Forms, followed by File → Import Form(s).
