We Already Filled Out This Referral Form. Why Are We Doing It Again?
If you've ever helped run a busy medical clinic, you probably know this frustration.
A physician completes a referral, the form is filled out, the fax is sent, and everyone moves on to the next patient. A few days later, the referral comes back:
"Sorry, we're no longer accepting that version. Please resubmit using our updated 2026 form."
Seriously? We just completed that form!
Sometimes the new version looks almost identical. A few fields have moved, a question has been added, or the hospital has changed its preferred layout. But for the referring clinic, that small change creates a surprising amount of work.
One Updated Form. A Whole Lot of Extra Work.
Working with a medical client, we've experienced this firsthand.
When a hospital or specialist updates a referral form, we don't just download a new PDF and call it a day. We need to retire the outdated form in the client's TELUS PS Suite EMR, build or import the replacement, align the fields, configure supported patient information to auto-populate, and make sure the finished document is easy for the client's physicians to complete.
Then we need to find the original patient referral, complete it again using the new version, and resend it. All because a form changed.
Now imagine managing 100, 200 or even 300 different Ontario hospital referral forms and requisitions. Different hospitals, different specialists, different updates, all arriving at different times throughout the year.
Individually, these aren't enormous technical challenges. Collectively, they consume administrative time — time that isn't spent caring for patients.
Helping a Medical Client Make Their EMR Work Harder
Rather than simply accepting all this extra work, we started asking a different question: if we're going to rebuild these forms anyway, how can we make them smarter?
Through Smart Clinic Systems, we've been refining TELUS PS Suite workflows with a medical client to make referral forms easier to complete, manage and send.
- Building professional auto-populating EMR forms that pull supported patient demographics and physician information directly from the EMR.
- Using standardized naming conventions so physicians can find the right PSS EMR forms quickly.
- Organizing the client's PS Suite address book so referral destinations and fax numbers can be populated more consistently, rather than repeatedly entered by hand.
- Making better use of the digital tools we already have, reducing unnecessary printing, repetitive typing and the effort of tracking down referrals.
These .cfm templates — often called CFM files — support the workflow; clinical selections and decisions remain with the physician. The goal isn't just to make a prettier form. It's to make the entire referral process a little easier, every single time.
Why We're Sharing Our Forms With Other Clinics
If we've already spent the time rebuilding a CHEO genetics requisition, an audiology referral or a specialist intake form for a medical client, why should another Ontario practice have to start from scratch?
They're facing the same challenges. So we're sharing ready-made TELUS PS Suite referral forms through Smart Clinic Systems.
We're expanding the library across CHEO requisitions, genetics, mental health intake, sleep medicine and other Ontario hospital and specialist forms. New additions are prepared, reviewed and tested before being offered for download. The searchable catalogue shows which forms are currently available — not every form we're working on is ready for sale.
We also help clinics convert existing PDFs into EMR-ready forms and update their templates when hospitals release new versions. Each clinic still confirms current hospital requirements and checks the form in its own PS Suite setup.
These aren't theoretical solutions. They're the same kinds of practical improvements we've been developing to make our medical client's clinic run more smoothly.
Better Forms. Less Admin. More Time for Patients.
Would a centralized, standardized referral system across Ontario make everyone's lives easier? Absolutely. Until we get there, hospitals and specialists will continue updating their forms, and referring clinics will continue adapting.
Our approach is simple: make those updates easier, build smarter forms, and share the work so clinics don't have to solve the same problem independently.
That's why we started building these tools for a medical client. And that's why we're sharing them through Smart Clinic Systems. Because keeping up with referral forms shouldn't be another full-time job.
